IMMUNIZATION
Florida requires certain vaccines to be administered before children may enroll and attend school. Each child must provide a Florida Certification of Immunization (DH 680 form), documenting the following vaccinations:

Florida requires certain vaccines to be administered before children may enroll and attend school. Each child must provide a Florida Certification of Immunization (DH 680 form), documenting the following vaccinations:
Public/Non-public Schools Kindergarten Through 12th Grade
(children entering, attending, or transferring to Florida schools)
- Four or five doses of DTaP vaccine
- Three doses of hepatitis B vaccine
- Three, four, or five doses of polio IPV vaccine
- Two doses of MMR vaccine
- Two doses of varicella vaccine
Additional Immunization Requirements for 7th Grade Entry
In addition to compliance with all other immunization requirements, children entering, attending, or transferring to the seventh grade in Florida schools must complete the following:
- One dose of tetanus-diphtheria-pertussis (Tdap) in grades seven through twelve
- An updated DH 680 Form to include Tdap, must be obtained for submission to the school.

SCHOOL ENTRY HEALTH EXAM
Students who are entering in Kindergarten, or the State of Florida, must provide evidence of a medical exam completed no less than 12 months prior to the child's school entry date. As long as the medical exam meets this 12-month requirement, parents may submit this information on the School-Entry Health Exam DH 3040 or provide a copy of the exam obtained from their current physician before moving to Florida.

SCOLIOSIS SCREENING [6TH GRADERS]
The Florida Department of Education requires a Scoliosis Screening for all 6th graders in accordance with Section 1003.22(4), Florida Statutes, and State Department of Health Rule 64F-6.003, Florida Administrative Code. Please have your child receive this screening through their General Practitioner or Pediatrician and return the Scoliosis Screening Release Form to our school office.
