Health Requirements

IMMUNIZATION

Florida requires certain vaccines to be administered before children may enroll and attend school. Each child must provide a Florida Certification of Immunization (DH 680 form), documenting the following vaccinations:

Public/Non-public Schools Kindergarten Through 12th Grade

(children entering, attending, or transferring to Florida schools)

• Four or five doses of DTaP
• Four or five doses of IPV
• Two doses of MMR
• Three doses of Hep B
• One Tetanus-diphtheria-acellular pertussis (Tdap)
• Two doses of Varicella (kindergarten effective with 2008–2009 school year, then an additional grade is added each year thereafter). Varicella vaccine is not required if there is a history of varicella disease documented by the health care provider.

SCHOOL ENTRY HEALTH EXAM

Students who are entering in Kindergarten, or the State of Florida, must provide  evidence of a medical exam completed no less than 12 months prior to the child's school entry date. As long as the medical exam meets this 12-month requirement, parents may submit this information on the School-Entry Health Exam DH 3040 or provide a copy of the exam obtained from their current physician before moving to Florida.

SCOLIOSIS SCREENING [6TH GRADERS]

The Florida Department of Education requires a Scoliosis Screening for all 6^th graders in accordance with Section 1003.22(4), Florida Statutes, and State Department of Health Rule 64F-6.003, Florida Administrative Code. Please have your child receive this screening through their General Practitioner or Pediatrician and return the Scoliosis Screening Release Form  to our school office.
